The Factors Influencing the ¡°Resting Energy Expenditure¡± of Massive Burn Patients

Abstract

|
Purpose: Accurate determination of energy needs in massive burn patients is very important, because underfeeding and overfeeding of energy are both associated with undesirable consequences. Postburn energy expenditure is affected by age, sex, burn size and depth, time after injury, inhalation injury, surgical treatment (early excision and graft), and sepsis. The purpose of this study was to analyze the factors influencing the ¡°Resting Energy Expenditure (REE)¡± of massive burn patients.
Methods: In 84 adult patients with more than 20% total body surface area burn admitted to the Hangang Sacred Heart Hospital between February, 2006 and December, 2006, REE was measured by indirect calorimetry. Sex, age, burn size, inhalation injury and sepsis were included in the factors which influencing the REE of massive burn patients. The REE was expressed as actual REE (kcal/day), REE/basal metabolic rate (BMR)(%), and REE/body mass index (BMI).
Results: The measured REE, REE/BMR and REE/BMI were significantly higher in males versus females (p£¼0.05). The measured REE and REE/BMI showed significant difference between age groups (p£¼0.05). The measured REE, REE/ BMR and REE/BMI showed significant difference among four groups, which were classified by area of burn (p£¼0.05). The measured REE, REE/BMR and REE/BMI for patients with inhalation injury were significantly higher than without inhalation injury (p£¼0.05). The REE/BMR for patients with sepsis was significantly higher than without sepsis (p£¼0.05).
Conclusion: Because of the REE of massive burn patients is affected by various factors, these should be considered for nutritional support in massive burn patients. We recommend indirect calorimetry measurements for determining resting energy expenditure in massively burned patients. (Journal of Korean Burn Society 2008;11:16-21)
